Continuum of Care - Ending Homelessness in Our Community

The Continuum of Care program, sponsored by the US Department of Housing and Urban Development, promotes the goal of ending homelessness in communities; provides funding for efforts by nonprofit providers and state and local governments to quickly re-house homeless individuals and families; promotes access to and effective utilization of programs; and optimizes self-sufficiency among those experiencing homelessness.

Current information on the program including its governing laws, regulations and forms can be found by visiting:

Continuum of Care Toolkit

At the local level, our regional Continuum of Care group consists of Cleveland, Gaston and Lincoln Counties. We are currently working to develop a Continuum of Care Plan specific to Lincoln County. Our goal is to provide a consistent process for identifying, assessing and assisting families, individuals, senior citizens, veterans and youth who are in need so they can get help immediately.
